• DOT#
• DL/CDL
• Copy of your insurance certificate and phone number for your insurance company
• Signed W-9 form
• Signed Dispatcher-Carrier Agreement & Power of Attorney (OPTIONAL)
• Company profile completed.
• Your factoring company’s name, address, and contact phone number (if applicable)
